Kelly Geffen, Divisional Director of Nursing for Walsall Healthcare Trust, has been invited to present at a nationally recognised webinar, showcasing the integrated work that has been taking place, as part of Walsall Together, to support admission avoidance and early discharge in Walsall.
The webinar, which is hosted by NHS England and NHS Improvement’s Discharge and Community Services team and led by Matthew Winn, Director for Community Health, will take place between 4pm-5pm this Friday 25 March.
Speaking live, Kelly will be focusing on the integrated assessment hub and the care navigation centre, highlighting the positive impact these services have had in keeping people out of hospital and cared for within their own home or as close to home as possible.
Kelly Geffen, Divisional Director of Nursing for Walsall Healthcare Trust, said: “It’s a real privilege to be invited to share the excellent work that has been taking place in Walsall to support our aim of reducing inequalities by providing care closer to home which is accessible for all who need it. Our services focus on using an integrated and mutli-disciplinary approach with health, social care, mental health, housing and the voluntary and community sector working together to help build resilience within our communities and promote well-being. This in turn should lead to a decrease in unnecessary hospital admissions and reduced lengths of stay for our patients.”
As well as hearing from Kelly there will also be presentations from Saf Angelo, NHS England’s Assistant Director for Rehabilitation, about the bed to home model and providing suitable care for people after hospital admission and Charlotte Wilson, Toby Hyde and Gina Naguib-Roberts on the Fuller stocktake and how can primary care and systems partners work together better.
